BIOGRAPHY

Dr. Chang is a Director/Attending Physician (M.D.) in the Division of Integrative Medicine, Center for Traditional Medicine, Taipei Veterans General Hospital, Taipei, Taiwan, and an Assistant Professor in the Faculty of Medicine, National Yang Ming Chiao Tung University (Taipei). Dr. Chang was awarded his Ph.D. from the Graduate Institute of Clinical Medical Sciences (GICM), Chang Gung University, Taoyuan, Taiwan. His Ph.D. study, The Efficacy of Chinese Herbal Formula on Sjögren's syndrome and its Mechanism of Anti-oxidative and Immune Modulatory Effects, reflects his ongoing fascination with the ability of TCM treatment to not only relieve but also modify the debilitating symptoms associated with Sjögren's syndrome, including gastrointestinal discomfort, poor appetite, xerostomia, and aphthous ulcer.

Dr. Chang’s innovative research extends beyond herbal medicine to integrated TCM approaches, such as acupuncture and herbal formulations. He has led groundbreaking clinical trials, including the application of TBDESJS (Chun-Yu-Ching-Hua-Yin, CYCHY), a threefold-concentrated herbal decoction that demonstrated significant improvements in tear secretion, reduced artificial tear usage, and enhanced sleep quality in patients with Sjögren's syndrome and dry eye syndrome. Furthermore, Dr. Chang initiated the ACUDES (Acupuncture for Dry Eye Syndrome and Sjögren's Syndrome) clinical trial, which explored the effects of precise acupuncture at GB20 (Fengchi) and BL2 (Zanzhu) points. The results showed substantial increases in tear secretion and remarkable symptom relief, highlighting the potential of acupuncture in treating chronic dry eye conditions.

Since graduating with his Ph.D., Dr. Chang has received numerous accolades recognizing his outstanding contributions to education and research, including the "Best Teacher Award," "Traditional Chinese Medicine Excellent Academic Research Award," and "Distinguished Clinical Teaching Physician Award" from institutions such as Taipei Veterans General Hospital, the Taipei Chinese Medical Association, National Yang Ming Chiao Tung University, and the National Union of Chinese Medical Doctors’ Association.

RESEARCH INTERESTS

Dr. Chang specializes in traditional Chinese medicine and rheumatology. His main research interests involve TCM clinical trials, Sjögren’s syndrome, systemic lupus erythematosus, systemic sclerosis, and bioinformatics studies. As a key developer of the novel Chinese herbal medicine SS-1 (a triple combination of Chinese herbal preparations consisting of extracts of Gan-Lu-Yin, Sang-Ju-Yin, and Xuefu-Zhuyu-Decoction), he has applied SS-1 in the clinic with very successful outcomes in patients with Sjögren's syndrome.

Building on his work with SS-1, TBDESJS (CYCHY), and the ACUDES acupuncture therapy, Dr. Chang has demonstrated a dedication to integrating TCM into modern medical research to provide holistic and scientifically sound treatments. He is particularly keen on being involved in research collaborations that include biotechnology, bioinformatics, biopharmaceutics, and innovations in integrative medicine.

中醫自體免疫暨乾眼症客觀量測實驗室 (Traditional Chinese medicine Autoimmune Disease and Dry Eye Syndrome Objective Measurements Lab，AIDDES Lab)致力於結合中醫理論與現代科技，進行前瞻性的研究，探索自體免疫風溼疾病與乾眼症患者的病理機轉及中醫輔助治療。AIDDES實驗室聚焦於透過客觀量測訊號，如中醫體質、舌診、脈診、虹膜、甲褶及心律變異等，加上DNA、RNA、蛋白質體學、口腔微生物菌相等生物醫學指標，深入探討客觀與生物醫學指標與自體免疫疾病的相關性，並進行跨領域的研究，以期建立更科學且精確的診斷與治療方法。

此外，我們也積極運用人工智慧及深度學習技術，對中醫體質、舌診、脈診資料進行自動化分析，並將其應用於處方的個人化設計。並希望可以通過基因序列、蛋白質體學、口腔微生物菌相…等研究，揭示自體免疫風溼疾病及乾眼症在分子生物學層面的機轉，並探索其中草藥及針灸等臨床介入方法的療效與潛力。我們的研究不僅有助於推動自體免疫疾病的精準醫療，還旨在構建真實世界的大數據研究，為未來的臨床決策提供有力支持。

因此，我們發起此捐款計畫，希望借助各界的支持，為患者帶來更好的醫療選擇，並推動中西醫結合。我們研究最終目標是幫助廣大的自體免疫疾病及乾眼症患者，減輕患者口眼乾燥症狀，提升生活品質，促進健康。

本專案將用於下列用途：

1.先進儀器設備的購置與升級：採購各類先進的研究儀器來提升實驗的精確度與效率，這包括心律變異係數分析儀、舌診及脈診儀、虹膜儀、甲褶鏡、基因定序……等設備，用於收集與分析患者的生理訊號與分子數據。

2.人工智慧與深度學習系統的開發與應用：我們計劃進行於人工智慧技術的開發，尤其是在中醫體質、舌診及脈診數據的自動化分析和中醫處方分析系統，目的是提高中醫診斷的準確性與中醫處方的精確度。我們將構建深度學習模型，並持續優化其性能，以應對複雜的臨床數據。

3.基因及多體學研究：為了探索自體免疫風溼疾病及乾眼症的病理機轉，我們將進行基因體、蛋白質體學、口腔微生物菌相等多體學研究，這不僅包括基因序列分析，還涉及RNA定序、蛋白質功能表現及口腔微生物菌相之變化。我們的目標是找出影響疾病發展的關鍵因子，從而發現新的治療靶點。

4.中草藥及針灸的臨床介入研究：我們將以嚴謹的臨床試驗設計，對中草藥及針刺療法在自體免疫疾病及乾眼症中進行療效分析與科學性驗證，中醫要擊針灸的大規模隨機對照試驗，可嚴謹評估中醫療法的有效性與安全性。

5.大數據研究平台的建立：為了促進長期數據的收集與分析，我們將建立自體免疫疾病暨乾眼症的的大數據研究平台，利用真實世界的數據進行病患狀況追蹤、療效評估及疾病預測。這將有助於我們對疾病進行全面的、長期的觀察，並為未來的臨床應用提供科學證據。

6.科研人才的培育與發展：我們希望藉由此捐款計畫，培育優良的碩博士學生與研究員培育，提供學術課程、研討會與研究資源，鼓勵他們投入中醫藥、針灸進行自體免疫疾病與乾眼症相關領域研究。我們也將與國際頂尖學術機構合作，促進國際交流與技術提升。

7.國際學術交流與成果發表：我們將積極參與國際學術會議，發表研究成果，並與全球學者進行交流。我們的目標是讓AIDDES實驗室的研究成果獲得全球認可，並推廣至更廣泛的臨床應用情境，造福更多的患者。

該如何理解「證」？-中醫證型現代研究的定性與定量

中醫常常強調「辨證論治」，「辨證」是「通過四診八綱、臟腑、病因、病機等中醫基礎理論，對病人表現的症狀、體徵進行綜合分析，辨別為何種證候」，而「論治」是「在辨證的基礎上，訂出治療措施」。因此需要有精確的辨證，才可以對應到適當的治則治法，再精準得出方藥來予以治療。不同中醫師辨證診斷的差異，往往只是由不同的視角看同一個證型。事實上「證」就是一種分類，辨證就是一種分類的方法，但目前對證的定性與定量始終沒有標準答案，可歸咎於兩個面向：1.中醫的證本身的基本定義不清楚（定性）。2.證型的量化程度不清楚（定量）。因此，近年來許多學者利用當時最先進的技術，莫不是要將證定義清楚並可被度量，使中醫複雜的證型可以透過簡單的幾個指標及測量值就可定義清楚與量化。因此知名替代與補充醫學領域的期刊Evidence-Based Complementary and Alternative Medicine (eCAM)，也在2012年針對「證」來提出探討，本文簡單論述中醫證型的相關現代研究，從生物資料學（bioinformatics）、基因體學（genomics）、代謝學（metabolomics）到實證醫學文獻回顧，疾病從糖尿病、失眠、肝癌到冠心病，在研究方法及疾病選擇上非常廣泛，雖然在現今講求臨床實證的條件下，中醫所做的努力仍不足，但也漸漸的、緩慢的往實　證療法邁進，不再只是停留在專書與老中醫之專家意見，相當值得鼓勵與學習。（臨床醫學 2014; 73: 189-95）

病態的社會

這是一個病態的社會，台灣現在的作法 (先聲明，我不是公務人員)：

公務員過很爽，民間過不爽，所以要把公務員拉下來，大家一起不爽

但是正面的作法應該是：

要跟公務員看齊，把大家薪水拉高，甚至要超過公務員

但是我沒看到老闆或是企業家有這樣的社會責任！

有得話就站出來呼籲吧！

重點不是作多久，是老闆要把員工一個人當兩個用，

事情當然做不完，老闆跟員工才會覺得做不完就加班。

但問題是，員工做不完就請兩個，

兩個人在五點下班，就可以去進修、購物、照顧小孩，

這不是最基本的嗎？所以是員工的問題嗎？

只要有牽涉到人的利益，就不是單純的統計問題

國際期刊邀請當Reviewer

這幾個月開始被國際期刊邀請當Reviewer，
有的有SCI，有的沒SCI，有的中文，有的英文，

但是剛剛收到一篇最奇怪，
是在探討某城市保險套在潮濕天熱的天氣下保存品質，
題目很怪，寫得很怪，本來想直接退，甚至回信說不想審，

結果突然看到是「某非洲小國」的研究，
我突然想到李教授曾經說過的一個故事，
強的國家做頂尖研究是最基本的事，因為有很多資源，教育環境也很好，
李教授曾經審到一個小國家的PAPER，
基於鼓勵性質，就盡量通過，
因為小國家連生存都有問題，怎麼談得到教育，更何況是研究，就算駔研究能跟先進國家比嗎？

所以我決定效法李教授，
再怎麼樣我都要Major revised，
雖然這篇沒SCI，
但我一定要鼓勵他們，

如果沒SCI都被退稿，那以後他們還會想做研究嗎？

看完PAPER我覺得很生氣，
他們已經無法控制生育力了，為什麼連保險套品質都很差！！
